GRADUATE THESIS FORMS
The following forms must be completed and submitted to the Graduate Coordinator
with Supervisor approval.
Thesis Supervisor Approval form: This
form is to be filled out and submitted at the end of the first term (M1).
Thesis Proposal Approval Form: This form is to be filled out and submitted at the beginning of the second term (M2). This is a one page abstract and title of your thesis proposal, approved by your Supervisor. The proposal may be changed over the course of study.
Thesis Committee Approval form: This form is to be filled out and submitted at the end of the second term (M2). A Committee must consist of your supervisor, two University of Waterloo Faculty, and a proposed External Member/Reader. The External Member is not contacted at this point, however a name may be provided. Students are not to have contact with this individual and arrangements are made by the Supervisor/Committee and Coordinator.
Draft Thesis Approval
form: This form is the final step towards defence. This form must
be completed at the final Draft Approval meeting. The defence date,
time, preferred room location, and External Member contact information
are noted. Please refer to the Defence Checklist below for detailed
timelines and requirements.
